Appointments may be booked in several ways.
Online
You can submit a request for an appointment online.
Your request will be assessed and assigned to the best person to help you with your concern. You may be seen by a doctor or other healthcare professional.
Nurse or healthcare assistant appointments
Routine nurse or healthcare assistant appointments are usually booked or arranged in advance, including routine vaccinations. Use the get help for any health problem form to request a routine nurse appointment.
Long term condition reviews
If you have a long term health condition we will invite you for a review in the practice every year. These conditions include asthma, COPD, diabetes, chronic kidney disease, dementia, cancer, hypertension or cardiovascular disease. It is important that you attend for your review so that we can monitor your condition and ensure that any medication that you are taking is still effective and appropriate for your condition. For some conditions, we will ask you to have a blood test prior to your appointment.
We will contact you when your long term condition (LTC) review is due. Appointments can be booked simply by:
- using the booking link that we send in the text message
- using the get help for any health problem form and select “routine care”

By phone
Patients can request an appointment by calling reception on 0121 550 4917.
In person
Patients can visit the surgery and request an appointment at reception.
